Fix several in call audio issues.
- 1: make sure that we restore normal mode in codec when the
call is ended.
- 2: make sure that we set in call mode in codec when the call
is started even if output stream is playing.
Merged support for special mic gain setting for voice recognition.
Some more cleanup.
