powerpc: Fix PPC_PTRACE_SETHWDEBUG on PPC_BOOK3S
Properly set the DABR_TRANSLATION/DABR_DATA_READ/DABR_DATA_READ bits in the dabr when setting the debug register via PPC_PTRACE_SETHWDEBUG.
